Department of Licensing Forms

 

Here are some important Department of Licensing forms:

 

A permit allowing restricted driving during a license suspension.  Not everyone qualifies for the TRL.

This form is required by the Department of Licensing for certain individuals that need an Ignition Interlock Device.

This form is critically important in protecting your privilege to drive for those arrested for DUI.  This form MUST be mailed within 30 days of your DUI arrest.

It is important to update your address with the Department of Licensing.  The Washington State DOL will mail important notices about your driving privilege.  Failure to respond to some notices may result in a driver license suspension.

This DOL form provides information about your driver record.  Traffic tickets, traffic crimes, license suspensions and reported accidents may be recorded by DOL.
